As we grow older, our health needs change. These changes can be gradual or come on suddenly after an accident or illness. At NWRH, we understand the challenges that come with ageing. We provide a range of services throughout Queensland to help manage your aged care requirements and maintain your independence for as long as possible.

We can assist you with registering for support through, My Aged Care. There are two main programs we work within. Through the Commonwealth Home Support Program, we provide basic home support services to assist you and your carer. We also provide help through the Home Care Package Program, which includes care for seniors with more complex needs.

National Disability Insurance Scheme (NDIS)

We are registered NDIS providers offering a range of support services to people living with disabilities, including assistance with daily activities, skill development, therapeutic supports, community participation opportunities and much more. Our services are designed to meet the individual needs of our clients, and we work closely with each client and their families to ensure our services suit their unique circumstances.

Every person is different, with their own strengths, abilities and goals. Our allied health professionals and care support workers understand this and will make sure to tailor our services accordingly. We are committed to providing respectful care and support to our clients. As a registered provider of support under the NDIS, we can provide Queensland participants with the following: 

  • Assistance with daily personal activities
  • Development of daily living and life skills
  • Participation in community, social and civic activities
  • Therapeutic supports
  • Assistive products for personal care and safety
  • Physical wellbeing activities 
  • Group activities 
  • Early childhood intervention

Therapeutic support can include occupational therapy and physiotherapy, through to speech pathology, dietetics, exercise physiology, podiatry and mental health services. To make an enquiry or learn more about the services we offer, please call our friendly team.

Queensland Community Support Scheme (QCSS)

If you’re not eligible for NDIS services however still require support due to your chronic health, mental health, or disability needs we are here to help you.

The Queensland Community Support Scheme (QCSS) provides support to individuals to maintain or regain your independence, to continue living independently in your own homes, or to participate in your community. 

Under QCSS we can offer a range of services including: Personal Care, Meal Preparation, Domestic Assistance, Capacity Building Activities and Community Connection.

You don’t need a referral to apply. Either contact the QCSS Access Point on 1800 600 300, or email us directly through our contact form to check if you are eligible for this program.

Community Transport

Do you have trouble accessing transport because of where you live? Or due to your financial situation?

We are here to help you.  The Community Transport Program supports people who are having trouble accessing transport options in their local communities.

My Aged Care is the Australian Government’s entry point for older Australians and their families seeking information on aged care services. The My Aged Care website provides a range of resources, including an online self-assessment tool and a directory of aged care services in your area. Our team at NWRH can help you understand the My Aged Care program and apply for assistance.

After you are approved by My Aged Care to receive services through the Commonwealth Home Support or Home Care Package, NWRH may not automatically be approved as your service provider. A service provider will be assigned depending on your needs.

My Aged Care is primarily funded by the Australian Government. If you are eligible for a home care package, you will have to pay a basic daily fee. Download your copy of the relevant fee schedule below:
