Support to navigate mental health and wellbeing services

At NWRH, our Front Door and Journey Coordination services are part of the MyndKind program, designed to help people access the right mental health and wellbeing support, at the right time.

We understand that navigating services can feel overwhelming. These services provide a welcoming, compassionate starting point, helping individuals and families connect with supports that meet their needs whether through MyndKind or other local services.
MyndKind is a stepped care model, meaning support is matched to what a person needs right now, and can change over time

  • Assist with referrals and service navigation
  • Support you to access the MyndKind program or other services

Journey Coordination

A Journey Coordinator is someone who works alongside you over time, helping you navigate services and stay connected to the supports you need.

They act as an independent partner in your care, focusing on building trust and helping you feel confident managing your wellbeing.

Journey Coordinators can:

  • Take time to understand your situation, goals, and needs
  • Help you explore and access the right services and supports
  • Coordinate care across different providers
  • Provide practical guidance, information, and advocacy
  • Support you through changes in your circumstances
  • Build your confidence to manage your wellbeing independently

Support is flexible and tailored – you may need short-term guidance or longer-term coordination.
